Self-pay:
- New patient is $350 and follow-up patient is $250
(includes traditional retina testing: OCT, OPTOS, Ultrasound, Visual Field, Lipiscan, Beacon—does not include fluorescein or ICG, iTrace, Oculus 5)
- We provide you with a "CMS 1500" form to submit directly for reimbursement to your insurance company back to you

Traditional Medicare:
- Includes comprehensive evaluation and one test on one date
- Could require other visits
Traditional Medicare (Membership)
- Includes multiple tests in one day
- Membership annual fee $500 not covered by insurance—covers multimodal testing same day, management of high-cost drugs for intravitreal injection billed to Medicare
